益生菌及其代谢物在调控结直肠癌中的研究进展

摘要: 结直肠癌是一种以基因突变累积和免疫反应失调为主要特征的肠上皮异质性疾病。结直肠癌的发生是环境、遗传等多种因素综合作用的结果。环境因素中的饮食因素与结直肠癌的发生关系密切。饮食因素在调节肠道微生物群中发挥重要作用,同时,肠道微生物组的组成及其代谢是结直肠癌发生的重要促成因素。特定肠道微生物如具核梭杆菌已证实与结直肠癌的发生呈现密切的相关性。调节肠道微生物群已成为预防和治疗结直肠癌的一种潜在手段。本综述回顾结直肠癌背景下的益生菌、下一代益生菌及其代谢物对结直肠癌的干预效果,重点讨论益生菌的各种代谢分泌产物,包括无细胞上清液、胞外多糖、短链脂肪酸、细菌素、细菌酶及胆汁酸和色氨酸代谢物对结直肠癌生物学作用的研究现状,以期为结直肠癌的临床治疗提供理论参考。

Abstract: Colorectal cancer is a heterogeneous disease of the intestinal epithelium characterized by the accumulation of genetic mutations and the dysregulation of immune responses. The occurrence of colorectal cancer is the result of a combination of environmental, genetic and other factors. Among environmental factors, dietary factors are closely related to the occurrence of colorectal cancer. Dietary factors play an important role in regulating the intestinal microbiota. In addition, the composition and metabolism of the intestinal microbiome are important contributors to the development of colorectal cancer. Specific intestinal microorganisms such as Fusobacterium nucleatum have been shown to be closely correlated with the occurrence of colorectal cancer. Regulating gut microbiota has emerged as a potential means of preventing and treating colorectal cancer. In order to provide a theoretical reference for the clinical treatment of colorectal cancer, this article reviews the intervening effects of probiotics, next-generation probiotics and their metabolites on colorectal cancer, with a focus on the current status of research on the biological effects of various metabolic secretion products of probiotics, including cell-free supernatants, extracellular polysaccharides, short-chain fatty acids, bacteriocins, bacterial enzymes, bile acids and tryptophan metabolites on colorectal cancer.
